COVID-19 Vaccine Information for Businesses
Thank you for your interest in learning about the COVID-19 vaccine. Please fill out this survey so that Columbia/Boone County Public Health and Human Services can keep you and your business updated on the local availability of vaccine. When the vaccine is available to you and your employees, we will be able to contact you with further instructions on where and how to get vaccinated.
Please note that we are following state guidelines on prioritizing populations to be vaccinated. Some employees may qualify sooner than others based on their age, job duties, living conditions, or health history. Filling out this survey does not guarantee you and your employees get a vaccine, but does provide a way for our department to contact you when doses are available.

Employers and their employees play an important role in helping reduce the spread of COVID-19 within their workplaces and the community. Columbia/Boone County Public Health and Human Services (PHHS) have created guidelines for businesses to help mitigate the COVID-19 virus and protect their staff and clients. Below are general guidelines that apply to all businesses.
- Do not allow sick employees to work.
- Practice social distancing to the greatest extent possible by maintaining a six foot distance between all individuals.
- Minimize face-to-face interactions.
- Eliminate nonessential travel.
- Limit capacity.
- Promote proper hygiene.
- Encourage employees to report any safety and health concerns to the employer.
- Post a sign at each entrance stating that individuals who have any symptoms of COVID-19 or recent exposure should not enter.
- Follow additional guidelines for all business meetings.
Business Guidance Toolkit
This toolkit provides an overview of the case investigation and contact tracing process during the COVID-19 pandemic. The toolkit is created to allow businesses to train themselves in these procedures so that they can contain any infections and exposures in the workplace.
